That's really stupid. There's nothing different about it except they use mario instead of sonic. Lame. I've noticed a lot of hacked games for emulators but I've never actually played any.

They mostly suck and are good for 30 seconds of their comedy value in some cases, but overall hacked emulator games are craptastically stupid.

Agreed... nothing really special about replacing the Sonic sprite with a Mario sprite.

what the hell you mean hacked? This is an actual pirated illegal game, not ur super KKK bros. For those who don't know, genesis is 16-bit, and NES is 8-bit let see if you could just texture swap, and it'll magically work.
Meh, some people really don't know their NES games.
little more info for ppl who don't know

Retrospec do great remakes of classic Spectrum, Amstrad, BBC, and Acorn amongst others.
Homebrew titles are great :D

Does Super Mario Wheelchair (or whatever it's called) fit into this category as well? It's just a sprite change of course, to make Mario look like he's moving around in a wheelchair.

Not really, because this is a actually game cartridge that was sold at retail stores. BIG DIFFERENCE from homebrew, a lot of people (not you) can't comprehend that fact tho.
